Licences alongside pharmacy permits
Who is allowed to treat me in El Mirage?
Ask for the treating person’s full name, licence type and licence number. Ask separately for the pharmacy’s name. Use those names when you check the public records.
Match the person to the right board
A licence is the state’s permission to practice a profession. The letters after a name tell you which board to use. The board is the public body that oversees that profession.
Public orders are written board decisions about a doctor, including decisions about wrongdoing.

What if the person providing remote care is elsewhere?
Registration permits this route for remote care. It is separate from holding a full Arizona licence. Other legal routes may apply too.
Telehealth means remote care. If you are sitting in El Mirage during the visit, ask what gives that person permission to treat you. Ask for their Arizona licence or registration details, then ask the board to confirm them.
Check the pharmacy separately
A pharmacy permit is the state’s permission for the pharmacy business to operate.
As of October 3, 2026, a pharmacy elsewhere sending medicine into Arizona needs an Arizona permit and permission where it is based.
Arizona checks a pharmacist’s personal licence separately from the pharmacy’s business permit, as of October 3, 2026.
Ask the service for the pharmacy’s full name and permit details. If you cannot find the permit record easily, ask the Arizona State Board of Pharmacy for confirmation.
Read Arizona’s pharmacy permit ruleWhat does a clear record prove?
Arizona restricts the board decisions and complaint records displayed on its websites, as of October 6, 2026. The main licence page may leave out older decisions.
A page showing no problems cannot rule out earlier complaints or decisions by the board.
Ask the board for past public decisions about that licence, plus public meeting notes and notices.
Older decisions may still appear in public meeting notes and notices. Details of an unfinished investigation may not be shared.
A licence check tells you about permission to work and public records. It cannot choose a treatment for you.
